Why this matters in perimenopause

Frozen shoulder has a nickname in orthopedics. They call it "the menopausal shoulder." That is not a coincidence.

Adhesive capsulitis disproportionately affects women between 40 and 60. Estrogen receptors exist in the shoulder capsule. When estrogen drops, inflammatory markers increase in the joint capsule, and the tissue thickens and contracts. One study found that women in perimenopause are five times more likely to develop frozen shoulder than men of the same age.

But here is the part that frustrates me. Most women do not know this. They wake up one morning unable to reach behind their back, and their doctor says "it will resolve in 18 months." True. It usually does. But those 18 months are brutal. And early intervention with the right shoulder exercises can cut that timeline significantly.

Resistance training also matters for bone density in the shoulder girdle. After menopause, the proximal humerus is a common fracture site. Progressive loading through shoulder exercises builds the mechanical stress that triggers osteocyte activity and maintains bone mineral density. It is not optional after 45. It is preventive medicine.
