Why this matters in perimenopause

The transition into perimenopause brings a significant decline in estradiol, which directly impacts musculoskeletal health. Estrogen is protective of our intervertebral discs and tendons; as levels drop, collagen synthesis decreases, leading to stiffer ligaments and a higher risk of 'creaky' joints. A 2018 study published in the journal Menopause found that lower estrogen levels are closely linked to increased disc degeneration in the lumbar spine among women compared to men of the same age.

Furthermore, the rise in cortisol—often dubbed the 'stress hormone'—during this life stage can exacerbate muscle tension in the trapezius and lower back. This creates a vicious cycle: hormonal stress leads to physical tightness, which then limits our range of motion. By performing specific movements, we help regulate the sympathetic nervous system and maintain muscle density. Strengthening the back is also a critical defense against osteopenia. Since the spine is a primary site for bone density loss, mechanical loading through resistance exercises is essential for stimulating osteoblast activity and keeping our structural frame resilient against the fractures that become a risk in post-menopause.
